Job summary

Qualifications

• Graduate of an accredited school of nursing with nursing degree or diploma.
• Current licensure to practice nursing in the state.
• CPR certification and valid driver’s license required.
• Experience in hospice care and community health preferred.
• Ability to supervise hospice aides and coordinate care with IDT.

Responsibilities

• Plans, organizes and directs hospice care for patients in home and community settings.
• Completes initial and ongoing assessments to determine hospice needs and care plans.
• Develops and revises plans of care with the interdisciplinary team.
• Administers medications and treatments per physician orders.
• Coordinates services with family/caregivers and monitors patient status.
• Provides patient and family education on health care, pain, and end‑of‑life care.
