Job summary

Ohio Living is seeking an experienced RN Field Staff - Hospice to plan, coordinate and deliver quality hospice care in patients' homes and healthcare facilities across Ohio.

You will assess patient needs, develop and implement comprehensive care plans, and manage pain and symptoms while educating families and coordinating with interdisciplinary teams. Travel, valid driver's license, and on-call availability are expected.

Qualifications

• Current unencumbered Ohio RN license required.
• At least 1 year of acute or medical-surgical nursing experience required.
• Medicare-certified home health experience preferred.
• Proficiency with Windows, Microsoft Office, and Internet required.
• EMR experience preferred; ability to document accurately and timely.
• Excellent reading, writing and spoken English.
• Valid driver's license with acceptable driving record.

• Care management and service delivery in patients' homes and facilities.
