Job summary

Three Oaks Hospice, Inc. is seeking an experienced Registered Nurse to administer skilled care for patients at their residences and coordinate with the interdisciplinary team. You will document care, educate patients and families, and ensure smooth operation of the care plan.

The role requires an RN license, CPR certification, and strong communication skills, with hospice or home health experience preferred. On-site work in Texas with a patient-centered, supportive culture is offered.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification is required.
• Proficient use of computers; MS Office experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patients’ physical, functional, psychosocial, and environmental needs.
• Implement the individualized plan of care and revise as necessary.
• Educate patients and families about disease process, end-of-life care, and self-care techniques.
• Provide nursing procedures and preventive therapies as appropriate.
• Coordinate care with the interdisciplinary team and refer as needed.
• Document progress and inform physicians of changes in patient needs.
• Assess caregiver ability and provide support as care evolves.
• Maintain orderly records and communicate using current organizational systems.
