Three Oaks Hospice in Suwanee, GA is seeking an experienced RN to administer skilled nursing care in patients' homes, coordinating with the inter-disciplinary team. This full-time role supports diverse ages and requires on-call rotation.

Qualifications

• RN license in state of Georgia
• Two years’ RN experience in a clinical care setting; hospice/home health preferred
• Current CPR certification required
• Proficient with computers and Microsoft Office
• Strong interpersonal and communication skills and ethics

Responsibilities

• Provide skilled nursing care to patients in their residence.
• Coordinate care with the interdisciplinary team and family.
• Educate patients and families on disease progression, self-care, and end-of-life care.
• Initiate and monitor the plan of care and document progress notes.
• On-call responsibilities and after-hours support as assigned.
• Assess patient needs and adjust care plans based on acuity and outcomes.
• Communicate changes to physicians and IDT members.
• Maintain compliance with hospice standards and quality measures.
