Job summary

Three Oaks Hospice is seeking a compassionate RN to provide skilled nursing care to patients in their homes across the Cumming/Dawsonville/Johns Creek/Suwanee area. You will coordinate with the interdisciplinary team and families to deliver comprehensive hospice services.

Responsibilities include assessments, care planning, family education, on-call coverage, and ensuring adherence to care standards and documentation. A valid RN license and CPR certification are required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal and communication skills; high ethics and compliance.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs.
• Implement the individualized plan of care and revise as necessary.
• Educate patient and family about disease process, self-care, end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing goal progress.
• Coordinate patient/family services with the IDT; prioritize needs.
• Inform physician and others of changes and outcomes of interventions.
• Determine scope/frequency of services based on acuity and needs.
• Assess caregiver capacity to meet patient needs; provide support resources.
• Communicate information using current processes and technology.
• Admit/re-certify hospice care criteria and levels of care.
• Perform on-call responsibilities and provide on-call service.
• Other duties as assigned.
