Job summary

Three Oaks Hospice in Georgetown/Hutto is seeking an experienced RN to deliver skilled nursing care in patients' homes. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to plan and manage care.

Responsibilities include comprehensive assessments, implementing the plan of care, educating families, documenting progress, and ensuring timely communication with physicians. Strong nursing skills, compassion, and reliability are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal and communication skills; ability to support patients and families effectively.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in their home environment.
• Implement the individualized plan of care and revise as necessary.
• Consult with and educate patients and families about disease processes, end-of-life care, nutrition and caregiving.
• Initiate necessary preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes reflecting progress toward goals.
• Coordinate patient/family services and prioritize needs with the IDT.
• Inform physicians and team of changes in patient needs and responses to care.
• Determine scope and frequency of services based on acuity and needs of patient/family.
• Assess caregiver ability to meet needs at admission and during care.
• Communicate information using current processes and technology.
