AccentCare Inc. is seeking a Hospice Registered Nurse (RN) to join our interdisciplinary care team in Middlesex, New Jersey. This PRN role offers $45.00-$50.00 per hour, based on experience, with comprehensive benefits and flexible scheduling.

You'll provide compassionate, patient-centered nursing care, develop and implement care plans, educate families, and coordinate with physicians and other professionals to ensure comfort and dignity at the end of life. On-call responsibilities may apply.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Provide individualized nursing care to maximize comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary team to create and carry out care plans.
• Perform comprehensive assessments to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately reflecting patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
