Job summary

VitalCaring Group is seeking a Hospice Registered Nurse (RN) to lead patient-centered hospice care in field settings, delivering comfort and dignity to patients at end of life. You will conduct comprehensive assessments, develop individualized care plans, and collaborate with a multidisciplinary team to ensure symptom control and compassionate support for families.

Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care.
• Ability to manage a caseload independently.
• Excellent communication in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: conduct comprehensive patient and family assessments; develop and manage individualized, comfort-focused care plans; deliver skilled nursing care with focus on pain and symptom management.
• Serve as the Care Coordinator: act as case manager, coordinating care across the interdisciplinary team; collaborate with physicians, social workers, chaplains, and aides; monitor patient condition and adjust plans to maintain comfort.
• Support Patients & Families: provide education, emotional support, and guidance; help families navigate decisions with empathy; build trusting relationships.
• Drive Quality & Compliance: maintain accurate documentation; participate in IDT meetings and care planning; support quality and compliance standards.
