Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education, patient advocacy, and coordinating interdisciplinary care to meet each family's end-of-life needs.

You will perform comprehensive assessments, develop and adjust care plans, administer medications as prescribed, educate patients and families, and collaborate with physicians, aides, social workers, and other

Qualifications

Graduate of an accredited school of nursing with RN licensure.
BSN preferred; hospice care experience preferred.
CPR certification required.
Excellent observation, communication, and problem-solving skills.

Responsibilities

Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
Develops and revises individualized care plans with goals and nursing diagnoses.
Delivers professional nursing care and administers physician-ordered medications.
Educates patients and families on self-care, medications, and plan of care.
Coordinates with interdisciplinary team and community resources to ensure continuity of care.
Participates in on-call duties and ensures necessary equipment and services are available.
