Queen City Hospice in Cincinnati, OH is seeking a compassionate Hospice RN Case Manager to lead the full nursing process from assessment to care planning and evaluation. You will deliver holistic patient care and coordinate with an interdisciplinary team to meet physical, emotional, social, and spiritual needs.

Qualifications

• Active RN license in Ohio (or compact state)
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of quality assurance and hospice care standards
• Valid driver’s license and auto insurance

Responsibilities

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
• Deliver holistic patient care addressing physical, emotional, psychological, social, and spiritual needs
• Collaborate with the interdisciplinary care team including physicians, social workers, and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update individualized plans of care
• Provide end-of-life care, including clinical pronouncement and family support
• Maintain accurate, timely documentation in compliance with state, federal, and company standards
• Supervise and oversee care delivery by Home Health Aides (HHAs)
