Day City Hospice in Dayton, OH and surrounding areas is seeking a compassionate Hospice RN Case Manager to guide patients and families through their hospice journey with expertise, compassion, and dignity.

Qualifications

• Graduate of an accredited RN program.
• Current Ohio RN license.
• Current CPR and First Aid certification.
• Valid driver's license with reliable transportation.
• Knowledge of quality assessment and performance improvement practices.

Responsibilities

• Manage and coordinate patient and family caseloads across the hospice care continuum.
• Conduct comprehensive assessments and determine patient care needs.
• Develop, implement, and update individualized care plans.
• Collaborate with physicians and interdisciplinary team members to ensure patient outcomes.
• Identify needs and facilitate appropriate interventions.
• Provide education, support, and counseling to patients and families.
• Coordinate services with community partners and healthcare providers.
• Complete accurate documentation and maintain regulatory compliance.
• Supervise and support Home Health Aides in delivering quality patient care.
• Provide end-of-life support and compassionate guidance.
