Job summary

Affinity Health Management in Indiana seeks an experienced Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The role emphasizes community health education and coordinating care with families.

The RN will complete comprehensive assessments, develop and revise care plans, administer treatments, and serve as Case Manager to ensure high-quality palliative care across assigned caseload. A valid RN license and CPR are required.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing.
• 1 year of nursing experience in the last 3 years, or a BSN with 2 years’ experience across public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, and math skills; nursing skills per competency checklist.
• Prolonged walking/standing; able to lift, position, and transfer patients; able to lift supplies and equipment.
• Must be a licensed driver with insured vehicle and CPR certification (hands-on check).

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family.
• Provides professional nursing care using the nursing process.
• Assesses and evaluates patient status and initiates or revises plan of care.
• Develops care plans with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs; provides health instructions.
• Assists with ADLs and promotes self-sufficiency and comfort care.
• Acts as Case Manager to coordinate patient care for assigned caseload.
• Participates in on-call duties and ensures needed equipment/services are available.
• Supervises ancillary personnel and pursues ongoing professional development.
• Engages in quality improvement activities as part of the team.
