Job summary

Heartland Hospice in York, PA seeks an experienced RN Case Manager to coordinate compassionate home-based end-of-life care. You will perform comprehensive assessments, develop and revise plans of care, and be the primary contact for patients and families.

You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains, educate caregivers, ensure documentation and compliance, and support quality improvement initiatives while delivering empathetic, patient-centered care.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Excellent communication and assessment skills.
• Ability to work with an interdisciplinary team.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions per plan of care.
• Develop and update plans of care and visit schedules.
• Coordinate with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease process and end-of-life care.
• Maintain accurate clinical documentation and ensure infection control.
