Morning Star Hospice and Palliative Care Solutions in York/Hanover, PA is seeking an experienced RN Hospice Case Manager to coordinate care for patients at home and in facilities, collaborating with an interdisciplinary team to develop personalized care plans and ensure comfort and dignity at end of life.

Qualifications

• Current and unrestricted RN license in PA.
• 1 year hospice or palliative care experience preferred.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
• Act as liaison between patients, families, and the interdisciplinary care team to ensure coordinated care.
• Educate patients and families on disease processes, medications, and end-of-life care options.
• Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
• Document patient care and progress in compliance with hospice regulations and company policies.
• Participate in regular team meetings to discuss patient status and plan of care.
