Qualifications

• RN nursing degree and licensure required.
• Minimum 2 years of relevant healthcare experience.
• Driver’s license required upon hire in Michigan.

Responsibilities

• Assess patients and plan individualized care to optimize comfort and symptom management.
• Coordinate and deliver end-of-life hospice care in home, facility, or hospital settings.
• Collaborate with physicians, social workers, and chaplains to support patients and families.
