Job summary

Well Care Health seeks a compassionate Hospice Registered Nurse Case Manager to provide end‑of‑life care and support to patients and families. You will assess needs, develop care plans with the hospice team, and educate families on hospice processes and coping strategies.

Qualifications

• Graduate of NLN accredited nursing program and licensed as RN in the state.
• Bachelor’s degree preferred.
• Current RN license and CPR certification required.
• Experience in hospice care preferred.

Responsibilities

• Complete initial, comprehensive, ongoing assessment of patient/family needs.
• Develop individualized plan of care with hospice team and family.
• Communicate with physicians about patient needs and changes.
• Coordinate holistic care with IDG and family; include bereavement support.
• Demonstrate mission, vision and values in daily work.
