Job summary

Corewell Health Hospice seeks a dedicated Hospice Nurse Case Manager to coordinate end‑of‑life care, assess patient needs, and develop individualized plans in collaboration with an interdisciplinary team.

You will travel to patients' homes, nursing facilities, and hospitals to provide direct care, documentation, and ongoing professional development to ensure compassionate, compliant hospice services.

Qualifications

• Associate’s degree in nursing is required.
• 2 years of relevant experience.
• Michigan RN license upon hire.
• Michigan Driver’s license upon hire.
• Bachelor’s degree preferred.
• 1 year of relevant experience preferred.

Responsibilities

• Coordinate and deliver end‑of‑life care and symptom management for patients and families.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, monitor symptoms, and adjust treatments for comfort.
• Collaborate with physicians, social workers, chaplains and other care team members.
• Provide education and support to patients and families about care options.
• Maintain compliant documentation of patient care and progress.
• Travel to patient homes, nursing facilities and hospitals to provide direct care.
• Engage in ongoing professional development and safety initiatives.

Education

Associate’s degree in nursing

Bachelor’s degree (preferred)
