Job summary

Corewell Health Hospice in Michigan seeks a Nurse Case Manager to coordinate end-of-life care for patients and families, under Nursing Manager supervision. You will assess needs, build care plans, and collaborate with an interdisciplinary team to deliver compassionate hospice services.

The role requires an RN license in Michigan, at least 2 years of relevant experience, and a valid driver's license. The schedule is full-time, day shift, 8:00 a.m.

Qualifications

RN license required in Michigan
2 years of relevant hospice/nursing experience
Valid Michigan driver’s license

Responsibilities

Assess patient needs and develop individualized care plans.
Manage and administer medications for symptom relief.
Collaborate with physicians, social workers, chaplains and other professionals.
Educate patients and families on disease progression and care options.
Document care activities and ensure regulatory compliance.
Travel to patients’ homes, nursing facilities, and hospitals for direct care.
Pursue ongoing professional development and participate in safety initiatives.
Perform other duties as assigned.
