Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice RN Case Manager to deliver skilled nursing care in patients’ homes, develop individualized care plans, administer medications, and educate patients and families.

As a faith-based, not-for-profit agency, we offer strong nurse-led leadership, meaningful 1:1 patient care, and opportunities for clinical growth within a national home care and hospice network.

Qualifications

• Active Michigan RN license.
• 1+ years of clinical nursing experience (Hospice preferred).
• Strong communication, assessment, and organizational skills.
• Compassionate, dependable, and mission-minded.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Deliver skilled nursing care in patients’ homes.
