Job summary

Optimal Care in Grand Rapids, MI, seeks a Hospice RN Case Manager to deliver skilled nursing and compassionate support to patients and families at home or in facilities, guiding care plans with dignity and quality of life.

You’ll coordinate with physicians and an interdisciplinary team, travel daily within North Grand Rapids and Newaygo counties, and build trusted relationships while practicing full scope of nursing in a meaningful end-of-life setting.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one year experience as a nurse in a clinical care setting.
• Reliable transportation and valid driver's license and automobile insurance.

Responsibilities

• Provide skilled nursing care to patients in their homes or facilities.
• Coordinate comprehensive care with interdisciplinary team.
• Build rapport with patients, families, and caregivers.
