Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice Registered Nurse Case Manager (RN) to deliver skilled nursing care in patients' homes across Grand Haven, Grand Rapids or Muskegon, MI. You will create individualized care plans, administer medications, and educate patients and families.

Requirements include an active Michigan RN license and 1+ year of clinical nursing experience (hospice preferred).

• Active RN license in Michigan is required.
• 1+ years of clinical nursing experience; hospice preferred.
• Strong communication, assessment, and organizational skills.
• Compassionate, dependable, and mission-minded; driver’s license and reliable transportation required.
