Qualifications

• Active RN license in Michigan (Hospice preferred)
• 1+ years of clinical nursing experience (Hospice preferred)
• Strong communication, assessment, and organizational skills
• Compassionate, dependable, and mission-minded
• Valid driver’s license and reliable transportation required

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient health.
• Educate patients and families about care and treatment.
• Coordinate with care team to ensure seamless services.
• Document patient progress and adjust plans as needed.
