Job summary

ProHealth Home Health and Hospice in Birmingham, AL seeks an experienced RN Hospice Case Manager to provide comprehensive nursing care to terminally ill patients at home and support families during bereavement.

You will work as part of the Hospice team to deliver coordinated care, onsite and through the IDG, ensuring effective symptom management, education, and continuity of care. A current RN license in Alabama is required.

Qualifications

• Graduate from an accredited school of nursing.
• Currently licensed as a registered nurse to practice in the state.
• Ability to work independently and make rapid, accurate judgments.
• Ability to supervise other staff appropriately.
• Experience as RN in hospice or related setting preferred; active patient contact in past three years preferred.

Responsibilities

• Regularly assesses and reassesses the nursing needs of the Hospice patient.
• Provides Hospice nursing services, treatments and preventive procedures.
• Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
• Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
• Attends team conferences and maintains records as required by Hospice.
