Job summary

Gentiva is seeking a Full-time Hospice Field RN Case Manager to serve patients in Jefferson County, AL. You will provide hands-on nursing care at patients' homes or in suitable settings, based on individualized care plans and in collaboration with an interdisciplinary team.

You will perform comprehensive assessments, document care, and educate families, supporting emotional and physical needs while maintaining regulatory compliance and quality care standards.

Qualifications

• RN license in the state of employment.
• 1+ year RN experience in hospice, palliative care, home health, or acute care preferred.

Responsibilities

• Serve as primary point of contact for patients and families in hospice.
• Deliver and document skilled nursing care per individualized Plan of Care at home or in alternative settings.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with IDG to develop and refine care plans.
• Educate patients and caregivers on disease, wellness, and end-of-life care.
