Job summary

Apploi is seeking an experienced Hospice RN Case Manager to provide skilled nursing care in patients’ homes and communities, focusing on comfort, dignity, and quality of life. You will develop individualized care plans, coordinate services with the IDG, and communicate clearly with patients, families, and healthcare teams in Beachwood and surrounding areas.

Qualifications

Current RN license in the State of Ohio.
Hospice, home health, or community nursing experience preferred.
Compassionate, organized, proactive, flexible, and dependable.
Strong clinical judgment and communication skills.
Ability to work independently while being a collaborative team member.
A genuine desire to provide exceptional end-of-life care.

Responsibilities

Provide skilled nursing assessment and care in patients’ homes and communities.
Develop and implement individualized plans of care.
Assess patient and family needs and coordinate appropriate services.
Collaborate with the interdisciplinary group (IDG).
Communicate effectively with patients, families, physicians, and team members.
Monitor medications, supplies, and equipment for appropriate utilization.
Document care accurately and maintain compliance with hospice regulations and standards.
Assist with On call requirements.
