Compassus in Georgia is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient-centered care in home and community settings. You will develop care plans, coordinate with families, and collaborate with the care team to balance comfort with independence.

The role requires an active RN license in Georgia, at least two years of nursing experience (hospice preferred), and CPR/BCLS certification.

Qualifications

• Active RN license in the state you work
• Minimum of two years of nursing experience (hospice experience preferred)
• Current CPR and BCLS certification
• Valid driver’s license

Responsibilities

• Provide compassionate, patient-centered hospice care and support to patients and families.
• Coordinate care plans with interdisciplinary team and community resources.
• Educate caregivers on care tasks and comfort measures.
