HOLY FAMILY HOSPICE AND SPIRITUAL CARE LLC in Georgia seeks a compassionate Hospice Registered Nurse (RN) to provide skilled nursing, case management, and patient/family education in home settings in accordance with Medicare guidelines.

The RN will coordinate care with the Interdisciplinary Group, manage pain and symptoms, supervise LPNs and aides, participate in on-call rotations, and adapt care to each patient’s home environment.

Formación

• Graduate of an accredited nursing program and active RN license.
• Current unrestricted RN license in South Carolina.
• CPR certification required.
• Minimum one year of clinical nursing experience.
• Hospice or palliative care experience preferred.

Responsabilidades

• Perform initial and ongoing comprehensive nursing assessments.
• Develop and revise individualized plans of care.
• Provide pain and symptom management.
• Administer medications and treatments as ordered.
• Coordinate care with all hospice disciplines.
• Educate patients and caregivers about hospice care.
• Supervise LPNs and Hospice Aides.
• Participate in IDG meetings.
• Complete documentation within 24 hours.
• Participate in on-call rotation as assigned.
• Adapt to patients' home environments.
