Job summary

Community Healthcare of Texas is seeking an RN Case Manager to coordinate hospice services, meet with patients and families, and manage the ongoing plan of care. You will administer medications, supervise LVN/CNA staff, participate in interdisciplinary group meetings, and handle on-call rotations to admit new patients.

• Texas RN license required.
• 1 year of nursing experience.
• Hospice case management experience preferred.
• Proficient computer skills (MS Office).

Responsibilities

• Meet with patients and families to explain hospice scope and plan of care.
• Coordinate admission of patients and ongoing care plans.
• Develop and revise care plans based on nursing diagnosis.
• Administer medications and treatments as prescribed.
• Supervise LVN/CNA care plans and delegate tasks.
• Attend interdisciplinary group meetings and participate in on-call rotation.
• Document clinical notes and report changes to physicians.
