Job summary

Community Healthcare of Texas is seeking an RN Case Manager to meet with patients and families, explain hospice services, coordinate admission, and develop ongoing care plans. This full-time position is based in Cleburne, TX, and requires a Texas RN license with 1 year of nursing experience and preferred hospice case management background.

Responsibilities include coordinating patient care, administering medications, documenting clinical notes, and collaborating with physicians and the hospice

Qualifications

• Texas RN license required.
• 1 year of nursing experience required.
• Hospice case management experience preferred.
• 2+ years hospice case management strongly preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provide nursing care using the nursing process.
• Assess and evaluate patient status and plan of care; reassess with patient/family.
• Develop goals-based care plans with patients and families.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care as the Case Manager with the clinical team.
• Maintain accurate and timely clinical notes and document changes.
• Communicate with physicians regarding patient needs and orders.
• Collaborate with family/caregiver and hospice interdisciplinary group in care planning.
• Participate in on-call rotation and assist with admissions.
• Supervise LVN/CNA care plans as needed.
