Job summary

Interim HealthCare Hospice & Palliative Care in Dayton, OH seeks a Hospice Registered Nurse Case Manager (RN) to oversee patient care plans and ensure timely, compassionate delivery of hospice services. You will collaborate with a multidisciplinary team to adjust plans as patient needs evolve, and provide direct, patient-centered care to support comfort and dignity at the end of life.

The role requires an RN license in Ohio, a relevant ADN/AAS (BSN preferred), CPR/FA certification, and reliable

Qualifications

• Associate degree in Nursing (ADN) or AAS required; BSN preferred.
• Current license to practice as a Registered Nurse (RN) in the state.
• Current CPR/FA certification.
• Reliable transportation to/from care sites and/or work locations.
• One year of professional RN experience in hospice or similar setting preferred.

Responsibilities

• Oversee implementation and ongoing assessment of the patient's plan of care with the team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers as needs evolve.
