Job summary

Bristol Hospice - State College LLC is seeking a compassionate RN Case Manager to provide whole-person, in-home hospice care in a patient- and family-centered setting. You will create individualized care plans, monitor conditions, and support families through the end-of-life journey with empathy and clinical excellence.

The role requires an active RN license in the state of employment, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Current CPR certification.

Responsibilities

• Develop and implement individualized care plans based on medical history and current needs.
• Provide attentive, one-on-one support to patients and families, ensuring comfort and dignity.
• Monitor vital signs and document changes; administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
