Job summary

Qualifications

Responsibilities

• Complete initial, comprehensive patient and family assessment for hospice needs.
• Provide professional nursing care per nursing process and Act.
• Develop, implement and revise plans of care with patient/family involvement.
• Administer medications and treatments as prescribed.
• Provide patient and family education and emotional support.
• Maintain clinical notes and communicate with physicians and care team.
• Attend interdisciplinary group meetings and participate in QI activities.
