Job summary

Qualifications

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Responsibilities

• Assume primary responsibility for a patient/family caseload including assessment, planning, implementation and evaluation of the nursing process.
• Initiate communication with attending physicians, other hospice staff and agencies to coordinate optimal care.
• Maintain regular communication with Director of Patient Services and attending physician regarding caseload and patient needs.
• Ensure coordination of care and resources for patients/families as part of the care team.
