CompassionCare Hospice in Reno, NV is seeking a dedicated Hospice RN to join our team. You will provide 1:1 patient-centered care in a compassionate setting, focusing on comfort and quality of life for patients and families in the Reno area.

As an RN Case Manager, you will assess needs, develop individualized care plans, administer medications, manage symptoms, and educate families while collaborating with an interdisciplinary team. A state RN license and CPR certification are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a reliable automobile and insurance.
• Possess and maintain current CPR certification in states where required.

Responsabilités

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
