Job summary

Paycom in Meridian, Idaho is seeking an RN Case Manager for Hospice to provide skilled nursing care in patients' homes under physician orders and the plan of care.

The RN administers intermittent visits, coordinates with the Interdisciplinary Team, and ensures care aligns with agency policies and patient needs. Licensure as an RN in Idaho is required, with 1 year of nursing experience preferred.

Qualifications

• Licensed RN in the state(s) of practice.

Responsibilities

• Provides services per the plan of care.
• Makes the initial evaluation visit and assesses patient needs.
• Develops the plan of care in coordination with the IDT.
• Regularly reevaluates nursing needs and revises the care plan.
• Initiates the plan of care and revisions as needed.
• Furnishes skilled nursing services and preventive procedures.
• Documents patient visits and care summaries per agency policy.
• Coordinates services with IDT, physicians, and caregivers.
