Job summary

Strong Force Staffing is assisting a well-established provider in Lewiston, ID, to fill an RN Case Manager, Home Health & Hospice role. The position is a W2, direct-hire opportunity supporting patient-centered care across home health and hospice settings.

• At least 1 year of experience in home health and/or hospice.
• Excellent communication and organizational skills.

Responsibilities

• Conduct thorough patient assessments and develop individualized care plans.
• Coordinate with interdisciplinary teams to ensure seamless care.
• Provide end-of-life care and symptom management for hospice patients.
• Educate patients and families on managing health conditions at home or during hospice.
• Maintain accurate and timely documentation for both home health and hospice services.
