Job summary

AuthoraCare Collective is seeking an experienced Hospice Registered Nurse to manage the overall plan of care for a caseload in collaboration with the interdisciplinary group (IDG). You will assess patient and caregiver needs, establish nursing goals and interventions, and work with physicians to develop the medical plan of care.

You will maintain therapeutic relationships, provide emotional support and teaching, document outcomes, and assume on-call responsibilities as scheduled; on-site care in

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Must hold a current RN license in North Carolina (NCBON in good standing).
• CPR certification required (AHA or Red Cross BLS).
• Valid driver's license and reliable transportation.

Responsibilities

• Manage the overall plan of care for hospice patients with IDG.
• Assess and identify patient/caregiver needs; set goals and interventions.
• Collaborate with physicians to establish medical care plans.
• Maintain therapeutic relationships and provide emotional support and teaching.
• Document outcomes and participate in on-call responsibilities.
• Provide dietary counseling as indicated.
