Job summary

Qualifications

• Current RN license in good standing.

Responsibilities

• Deliver and document skilled nursing care per the Plan of Care in patient homes or other living settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
