Job summary

Yale New Haven Health Services Corp. is seeking a Hospice Registered Nurse Case Manager to join the interdisciplinary team. The role focuses on compassionate, comprehensive end-of-life care, developing the patient plan of care, coordinating with providers, and ensuring safe delivery of services in the patient's home.

The RN will oversee CMS data collection and maintain documentation in the EMR, while collaborating with physicians and staff to enhance quality of life for patients.

Qualifications

• Nursing Diploma or Associate Degree in Nursing required; Bachelor's degree preferred.
• Minimum of two years nursing experience; home care/hospice experience preferred.
• RN with Connecticut license required; valid driver's license and car insurance if using personal vehicle.
• CPR certification required; obtain within 3 months of hire if not held.
• Understanding of Medicare certification and CMS data collection tool documentation experience.
• Strong organizational skills and ability to work independently; strong computer skills.
• Ability to manage a variety of assignments while prioritizing tasks.

Responsibilities

• Assumes primary responsibility for assigned patients and performs all phases of the nursing process.
• Assesses/reassesses patients and establishes patient-specific care plans.
• Collaborates with providers and other disciplines and communicates changes promptly.
• Educates patients/caregivers about hospice philosophy, disease progression, and care needs.
• Completes EMR documentation within 24 hours with changes and follow-up details.
• Oversight of LPNs and Home Health Aides.
• Reviews and updates the POC per regulatory standards.
• Schedules patients independently according to the plan of care.
• Participates in IDT, case management conferences and documents per guidelines.
• Attends staff meetings and trainings; participates in after-hours coverage as needed.
• Coordinates and manages care of patients receiving services in their home.
