Job summary

Lighthouse Hospice Care LLC is seeking an RN Case Manager to provide nursing care in patients' homes across Bridgeview, Burr Ridge, Palos Hills and Orland Park. The role coordinates care with an interdisciplinary team, families, and referring agencies to ensure high-quality end-of-life care.

The ideal candidate holds an RN license with a nursing degree and has experience in clinical settings; hospice experience is preferred. A valid driver’s license and willingness to travel locally are required.

Qualifications

• RN license required and graduate from an accredited nursing program.
• Hospice experience preferred but not required.
• Excellent reading, writing and comprehension of English.
• Must have valid driver’s license and be able to travel locally.
• Prolonged standing and walking; ability to lift up to 50 lbs.

Responsibilities

• Perform initial and ongoing assessments of patients in their homes to determine care needs.
• Coordinate care with the Interdisciplinary Group/Team and family members.
• Develop and update the plan of care to ensure quality and continuity of care.
• Educate patients and families about disease process, self-care, and end-of-life options.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care.
• Document progress notes and communicate changes to physicians.
• Monitor cases for payer requirements and maintain compliance.
• Participate in Hospice QAPI programs and promote agency policies.
