Job summary

Lighthouse Hospice Care is seeking a Hospice RN Case Manager to coordinate comprehensive home-based nursing care for patients across Bridgeview, Burr Ridge, Palos Hills, and Orland Park, IL. The role emphasizes compassionate end-of-life care and collaboration with an interdisciplinary team.

Responsibilities include assessing patient needs, planning and documenting care, educating families, and ensuring continuity of quality service while meeting payer requirements.

Qualifications

• Graduate from an accredited nursing program (Diploma, Associate or Baccalaureate).
• Current state RN license and driver’s license.
• Hospice experience is preferred.
• Experience as an RN in a clinical care setting.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the patient and needs.
• Develop, implement, and document the plan of care and adjust as needed.
• Educate patients and families on disease, self-care, and end-of-life care; coordinate with IDG/Team.
• Provide clinical direction to Hospice Aide and LPN/LVN to ensure quality care.
• Communicate changes in patient needs to all stakeholders and document progress.
• Monitor cases for third-party payor requirements and ensure compliance.
