Job summary

Mission Healthcare is seeking a registered nurse to provide comprehensive home health services in Carson City and surrounding areas. The role includes initial patient assessments, care planning, and ongoing evaluation to adapt to changing needs.

The ideal candidate holds an RN license, at least 1 year of acute care experience, BLS, and a valid driver's license. Home health experience is preferred; a Bachelor's degree in Nursing is required.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Bachelor's degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and revises as patient status changes.
• Regularly re-evaluates patients' needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
