Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing role in Jefferson Hills, PA, providing end-of-life care to patients in their homes. You will travel within the assigned territory and deliver compassionate care as part of an interdisciplinary team.

The position requires an active RN license in Pennsylvania and at least two years of RN experience; hospice experience is preferred. A valid driver’s license and approximately 20% travel are required.

• Two years of RN experience with hospice care preferred.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
