You'll assess patients, manage symptoms, and collaborate with an interdisciplinary team to deliver compassionate end-of-life care in a home setting, requiring a valid RN license and driving as needed.

Qualifications

• Active RN license in the state you work
• Two years of RN experience with clinical patient care
• Hospice experience preferred but not required

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning
• Educate and support patients and families through the end-of-life journey
• Ensure compliant EMR documentation and coordinate with the interdisciplinary team
