Qualifications

• Two years of RN experience
• Active RN license in the state you work
• Current driver’s license and ability to travel ~20% of the day

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team
• Perform comprehensive patient assessments and end-of-life care planning
• Educate and support patients and families through the end-of-life journey
• Maintain thorough EMR documentation
• Travel to patient homes within the assigned territory
