Three Oaks Hospice is seeking a dedicated Registered Nurse to provide skilled nursing care for patients at home, coordinating with an interdisciplinary team to ensure high-quality hospice services. The role emphasizes assessment, care planning, and family education in end-of-life care.

Responsibilities include updating care plans, performing home visits, coordinating services, and participating in on-call coverage as needed.

Qualificações

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and a valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsabilidades

• Perform initial and ongoing assessments of the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care and revise as necessary in collaboration with the care team.
• Educate patient and family about disease process, end-of-life care, nutrition, and self-care; provide staff training as needed.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Prepare clinical and progress notes showing progress toward goals and coordinate all patient/family services with the IDT.
• Provide on-call service to patients/families as assigned and communicate changes to physicians.
