Job summary

Affinity Hospice Holdings, LLC is seeking a Nurse Practitioner to provide hospice medical care under a collaborating physician. You will perform histories, exams, and assessments, diagnose conditions, prescribe treatments, and coordinate with families and the care team to plan services.

PRN hours may apply, and you will visit patients to evaluate eligibility and deliver compassionate, patient-centered care.

Qualifications

• Master’s degree required or equivalent in nursing; hospice experience preferred.
• One year NP experience minimum; state NP licensure required.
• CPR certification; CHPN or similar certification preferred.
• Accreditation from an approved certifying body for advanced practice nursing.
• Driver’s license and access to insured vehicle as required.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete history, exam, and documentation; order/interpret tests as needed.
• Develop individualized care plans; prescribe meds and coordinate hospice services.
• Meet hospice face-to-face visit requirements for eligibility periods.
• Provide information to the hospice medical director for eligibility decisions.
• Collaborate with interdisciplinary team to review and revise plans of care.
• Ensure symptom control and adherence to state regulations.
