Job summary

Affinity Care of Ohio is seeking a Registered Nurse to provide compassionate hospice home health care. You will perform comprehensive patient and family assessments, develop and update care plans, and administer prescribed treatments in collaboration with an interdisciplinary team.

The role emphasizes patient-family counseling, education, and ensuring safe, supportive care in home settings, with on-call responsibilities and supervision of aides as needed.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum of 1 year of experience as a professional nurse in the last 3 years; OR a BSN with 2 years of experience, at least 1 in public health/home care/hospice.

Responsibilities

• Conducts initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates care plans; revises plans as patient status changes.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing needs and self-care; supports activities of daily living.
• Maintains accurate clinical notes; communicates with physicians about patient needs.
• Coordinates care with the hospice interdisciplinary team and family/caregivers.
• Engages in on‑call duties and ensures availability of equipment and services.
• Supervises hospice aides when required; participates in quality improvement activities.
