Job summary

Visiting Nurse Association of Wisconsin, Inc. is seeking a compassionate Registered Nurse to coordinate end-of-life care for terminally ill patients and families across hospice settings.

You will lead the care plan, educate patients and families, and ensure timely communication with physicians and the IDG. The role requires 1–2 years in nursing with home health, hospice, medical/surgical or oncology experience; Wisconsin RN license and BLS; strong assessment, documentation, and teamwork.

Responsibilities

• Coordinate patient/family care for terminally ill patients in hospice settings.
• Educate patients and families on disease processes, management, and end-of-life care.
• Review patient caseload and ensure appropriate level of care and referrals.
• Lead the interdisciplinary care group (IDG) and develop/modify care plans.
• Document accurately in the electronic health record and communicate changes to the team.
• Supervise home health aides and licensed practical nurses per policy.
• Facilitate discharge planning and connect with community resources.
