Job summary

Continuum Hospice is seeking Per Diem Nurse Practitioners to deliver compassionate hospice care in and around the Barnstable area. You will assess health histories, diagnose conditions, and develop personalized care plans in collaboration with patients, families, and practitioners.

The role involves per diem scheduling, with responsibilities spanning education, care coordination, and adherence to Medicare/Medicaid guidelines. Benefits and onboarding support are provided.

Qualifications

• Master's degree in nursing; hospice/palliative care experience preferred.
• Current RN and NP licensure in the state of practice.
• CHPN or similar hospice/palliative certification preferred.

Responsibilities

• Deliver comprehensive medical care and palliate for terminal illnesses under physician supervision.
• Collaborate with attending physician, hospice medical director, and other practitioners for eligibility and ongoing care.
• Coordinate care with the interdisciplinary team to ensure patient-centered planning and delivery.
